Immigration Medical Exams
At Hayat Clinic, we provide complete USCIS-authorized Immigration Medical Exams (IME) for individuals applying for Green Cards and other immigration-related statuses. Our certified Civil Surgeon, Dr. Sabir, is approved by USCIS to perform immigration physicals and complete Form I-693 accurately and efficiently.

- Physical exam by a USCIS-certified Civil Surgeon
- Required blood and urine tests (Syphilis and Gonorrhea)
- Tuberculosis (TB) screening via IGRA blood test
- Review of vaccination history
- Completion and sealing of Form I-693

Who Needs an Immigration Medical Exam?
- Green Card (permanent residency) applicants (Form I-485)
- Adjustment of status applicants
This exam must be performed by a USCIS-approved Civil Surgeon, like Dr. Sabir at Hayat Clinic.
Required Vaccinations for Immigrants
- Measles, Mumps, and Rubella (MMR)
- Polio
- Tetanus and Diphtheria (Td or Tdap)
- Hepatitis B
- Varicella (Chickenpox)
USCIS requires proof of the above vaccinations. If you do not have proof, we can provide the needed vaccines (additional charges apply). Please bring official vaccination records to your visit. If not in English, a certified translation is required.
USCIS-Required Lab Testing
- Syphilis – RPR blood test
- Gonorrhea – Urine test
- Tuberculosis (TB) – IGRA blood test (QuantiFERON® or T-SPOT®)
A positive TB result requires a chest X-ray, which may incur additional charges.
Form I-693 Completion
After your exam is complete:
- You will review and sign your section of Form I-693
- Dr. Sabir will complete and certify the civil surgeon portion
- Your documents will be sealed in a USCIS-approved envelope
Do not open the sealed envelope, as it must be submitted to USCIS unopened.
